How to Register for Patient Access

Patient Access is a great online service which allows you to book appointments, order repeat prescriptions, view your medical records and send secure messages to the practice. The services is open 24/7/365 and can be accessed from your home PC, Tablet or Mobile phone.

Register at the Surgery in Person

Step 1: Read the information leaflet and familiarize yourself with the service details.

Step 2: Complete the ‘Patient Access Application Form’. Download, fill out and print it.

Step 3: Visit the Practice: Bring your completed form to the reception. Let the receptionist know you’ve read the information leaflet.

Step 4: Provide Identification. One photo ID such as passport, drivers license or freedom pass. For more information, here is a Full List of acceptable ID.

After completing these steps, the receptionist will provide you with a Patient Access account, granting you full access to all requested online services.

Note: You can also obtain the application form and leaflet as the reception if needed.


Register From Home and Visit the Surgery Later

You can also register for this service online without visiting your practice by clicking here.

This will create a basic account that allows you to book one appointment. After registration, you must visit the surgery in person and bring two forms of ID:

One photo ID (passport or drivers license) and one document showing your home address (recent utility or bank statement).

To register, click the GREEN button below. After clicking the button, you will be asked the following question: 

You will be asked, ”Have you received a registration letter from your practice?” Choose ”NO” and complete the subsequent questions.

At the end of your registration, please make a note of your USER ID, as you’ll need this to access your Patient Access Account.

Remember: once you’ve registered yourself online, you will need to bring in your ID so that we can give you full access to your account. You will also need to read our information leaflet and download, complete and print our ‘Patient Access Application Form’
